Liver Biopsy

Liver Biopsy
When is this procedure recommended by a gastroenterologist?
This procedure is commonly recommended to investigate, diagnose, or manage symptoms such as:
Unexplained liver disease when FibroScan and blood tests are inconclusive
Suspected autoimmune hepatitis — where biopsy defines treatment
Monitoring treatment response in hepatitis B or C
Suspected drug-induced liver injury (DILI)
Pre-transplant liver assessment or Wilson's disease workup
Unexplained portal hypertension

Expert Advice
Dr. Akhilesh Yadav
Understanding the Clinical Value of Liver Biopsy
Dr. Akhilesh Yadav reserves liver biopsy for situations where non-invasive tests cannot provide the diagnosis needed to guide treatment. With FibroScan and advanced blood markers, biopsy is now needed far less often — but when it is required, it provides definitive tissue-level diagnosis that changes treatment decisions.
Timeline Overview
The end-to-end patient journey for Liver Biopsy.
1. Preparation (पहले)
Fasting for 6-8 hours, adjusting medicines, and reaching with a companion.
2. Procedure (दौरान)
15-30 minute check under comfortable sedation; direct HD imaging & painless biopsy if needed.
3. Recovery (बाद में)
Wake up comfortably, rest in daycare for 2 hours, and return home with report.
Liver Biopsy
Important checklist parameters before your scheduled procedure.
Fasting required for 6 to 8 hours before the procedure. Do not eat or drink anything after midnight if the biopsy is scheduled in the morning.
2. Diet Guidelines (खान-पान)
Light meal the night before. Avoid spicy or heavy food. Do not drink alcohol for at least 48 hours before the procedure.
3. Regular Medicines (दवाइयाँ)
CRITICAL: Stop blood thinners (Aspirin, Clopidogrel, Warfarin) 5-7 days in advance. Stop NSAIDs (Ibuprofen, Diclofenac) 3 days before. Inform Dr. Yadav about all medications, including herbal supplements.
4. Documents (आवश्यक कागजात)
Bring: blood test reports (CBC, PT/INR, LFT, KFT), ultrasound reports, hepatitis B/C reports, FibroScan result, current prescription list, and a government ID.
5. Family Member (साथी/अभिभावक)
Mandatory — you must bring a family member or companion to assist with transport home. You will not be allowed to drive for 24 hours.

Walkthrough of the clinical steps taken during a Liver Biopsy procedure.
Pre-Biopsy Blood Tests
Platelet count and PT/INR are checked to ensure clotting is safe. A platelet count > 60,000 and INR < 1.5 are typically required.
Ultrasound Marking
Dr. Yadav performs a bedside ultrasound to identify the ideal liver entry point — typically in the right lower rib area — and marks the site.
Local Anaesthesia
The skin and tissue over the marked site are numbed with local anaesthesia (lidocaine). You may feel a brief sting, then numbness.
Biopsy Needle Pass
Using ultrasound guidance, a special biopsy needle is passed into the liver in a single, rapid motion (< 1 second) to collect a thin liver tissue core. You will be asked to hold your breath momentarily.
Observation
You rest under monitoring for 4-6 hours. Pulse, blood pressure, and pain are checked regularly. A post-procedure ultrasound confirms there is no internal bleeding.
Recovery Guide
What to expect and how to care for yourself after a Liver Biopsy procedure.
What to Expect
Most patients feel a mild ache or pressure at the biopsy site for a few hours. Serious pain, significant bleeding, or fever are uncommon but require immediate medical attention.
When to Return Home
Discharge typically in 4-6 hours once vitals are stable and post-procedure ultrasound is clear. Rest at home for the remainder of the day.
Diet Guidelines
Resume light diet (khichdi, dal, rice) in the evening. Normal diet from the next day. Avoid alcohol for 1 week.
Physical Activity
Complete rest for 24 hours. Avoid heavy lifting, vigorous exercise, and driving for 7 days.

Key Benefits of the Procedure
Procedural Risks
Liver biopsy is a safe, well-established procedure when performed by an experienced gastroenterologist. Serious complications (major bleeding, infection) occur in less than 1 in 500 procedures. Dr. Yadav uses ultrasound guidance to minimise risk.
Success Rate
Diagnostic-quality tissue is obtained in over 95% of biopsies performed by an experienced hepatologist.
Continuous Monitoring
Continuous pulse oximetry and blood pressure monitoring during and for 4-6 hours after the procedure. A post-procedure ultrasound is performed before discharge.
Frequently Asked Questions about Liver Biopsy procedures.
Doctor's Personal Advice
"We always try to avoid liver biopsy when non-invasive tests like FibroScan and blood markers can give us the answer. But for autoimmune liver diseases, certain treatment decisions, and pre-transplant workups, biopsy provides information that no other test can. When we do perform it, ultrasound guidance and careful patient selection make it very safe."
